Change the configured serial console index to reflect that it is the
first physical UART, reorder some defconfig lines and move some
environment configuration from bcmstb.h to bcm7445.h.

We have the following cases:
- CONFIG_NR_DRAM_BANKS was defined, migrate normally
- CONFIG_NR_DRAM_BANKS_MAX was defined and then used for
CONFIG_NR_DRAM_BANKS after a check, just migrate it over now.
- CONFIG_NR_DRAM_BANKS was very oddly defined on p2771-0000-* (to 1024 +
2), set this to 8.

Add support for loading U-Boot on the Broadcom 7445 SoC. This port
assumes Broadcom's BOLT bootloader is acting as the second stage
bootloader, and U-Boot is acting as the third stage bootloader, loaded
as an ELF program by BOLT.
