161786259c usb: Drop dm.h header file
This header file should not be included in other header files. Remove it
and use a forward declaration instead.

Also move the inline function out into a C file. We should not include C
code in headers.

fd09c205fc usb: s/CONFIG_DM_USB/CONFIG_IS_ENABLED(DM_USB)/
This allows to disable the USB driver model in SPL because it checks
the CONFIG_SPL_DM_USB variable for SPL builds. Nothing changes for
regular non-SPL builds.

faa7db24a4 usb: Move determination of TT hub address/port into separate function
Start split and complete split tokens need the hub address and the
downstream port of the first HS hub (device view).

The core of the function was duplicated in both host/ehci_hcd and
musb-new/usb-compat.h.

bcbe3d1579 dm: Rename dev_get_parentdata() to dev_get_parent_priv()
The current name is inconsistent with other driver model data access
functions. Rename it and fix up all users.

e740ca3cab musb: Update usb-compat to work with struct usb_device without a parent ptr
When building with CONFIG_DM_USB=y struct usb_device does not have a parent
pointer. This commit adds support to the musb code to deal with this.

b918a0c6f6 musb-new: Properly remove a transfer from the schedule on timeout
If a transfer / urb times-out, properly remove it from the schedule, rather
then letting it sit on the ep head. This stops the musb code from getting
confused and refusing to queue further transfers after a timeout.

Tested by unplugging a usb-keyboard, replugging it and doing a usb-reset,
before this commit the keyboard would not work after the usb-reset.

eb81955bf0 musb-new: port of Linux musb driver
Existing U-Boot musb driver has no support for the new gadget framework
and also seems to have other limitations. As gadget framework is ported
from Linux it seems pretty natural to port musb gadget driver as well.

This driver supports both host and peripheral modes.

This is not a replacement for current musb driver (at least now) as
there are still some consumers of the old UDC interface.

No DMA operation support included, CONFIG_MUSB_PIO_ONLY should be
defined.

Virtual root hub device is not implemented.

Known problems: with no devices connected usb_lowlevel_start() fails.
