At present when a file is bound to a host device it is always marked as
removeable. Arguably the device is removeable, since it can be unbound at
will. However while it is bound, it is not considered removable by the
user. Also it is useful to be able to model both fixed and removeable
devices for code that distinguishes them.
Add a -r flag to the 'host bind' command and plumb it through to provide
this feature.

This function has a strange mix of declarations and argument parsing
which is a bit hard to follow and harder to modify. Separate out the
declarations at the start of the function and adjust the ordering of
the code slightly.

When trying to bind to a non-existent file the following output is
displayed:
=> host bind 0 non_existent
Failed to access host backing file 'non_existent'
exit not allowed from main input shell.
The last line is quite unexpected and due to an incorrect return value.
If do_host_bind() fails, return CMD_RET_FAILURE (= 1).

We should not use typedefs in U-Boot. They cannot be used as forward
declarations which means that header files must include the full header to
access them.
Drop the typedef and rename the struct to remove the _s suffix which is
now not useful.
This requires quite a few header-file additions.

At present dm/device.h includes the linux-compatible features. This
requires including linux/compat.h which in turn includes a lot of headers.
One of these is malloc.h which we thus end up including in every file in
U-Boot. Apart from the inefficiency of this, it is problematic for sandbox
which needs to use the system malloc() in some files.
Move the compatibility features into a separate header file.

With the patch below applied, host_block_dev structure was switched
to be placed in platdata rather than priv. The command "host info"
must be aligned with this change. Otherwise, we will see "Segmentation
Fault."

The old 'sb' command was deprecated in 2015 and replaced with 'host'.
Remove the remaining users and the command, so that the name is available
for other purposes.

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.
In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.
This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

Update the host driver to support driver model for block devices. A future
commit will remove the old code, but for now it is useful to be able to use
