KVM: PPC: Replace SPAPR_TCE_SHIFT with IOMMU_PAGE_SHIFT_4K 

SPAPR_TCE_SHIFT is used in few places only and since IOMMU_PAGE_SHIFT_4K
can be easily used instead, remove SPAPR_TCE_SHIFT.
